Here is the stock Shift Knob.  It's kinda ugly but it gets the job done.
To remove the shifter you must first seperate the boot from the knob.  Just pull down really hard to seperate them.
The stock shifter just unscrews off.  Unscrew the shifter to remove it.
Now I have the aftermarket Momo shift knob in hand.
The bottom unscrews to reveal 3 allen wrench screws.
You want to unscrew the screws a bit so it will fit on the car.
First place the screw off bottom on, then put the knob on top of that.
Now just push down on the shift until it stops.  You can try to screw it in there too.  Next just tighten the allen bolts evenly.  Make sure they all get really tight.
Now screw the bottom piece up to hide the threads and allen bolts.

 The finished product.
